The agreement between you and Copergrine Health & Wellness when you book or receive care.

These Terms of Service ("Terms") govern your access to and use of services provided by Copergrine Health & Wellness ("Copergrine," "we," "us," or "our"), including our website at health.copergrine.com, our patient portals, our in-person clinic in Houston, Texas, and our telehealth services.
By booking an appointment, creating an account, providing your payment method, or receiving care from us, you agree to these Terms and our Privacy Policy. If you do not agree, please do not use our services.
You must be at least 18 years old to book an appointment for yourself. Minors must be booked by a parent or legal guardian who agrees to these Terms on their behalf.

When you book an appointment, you must provide a valid credit or debit card. We will save your card securely via Stripe (PCI DSS Level 1 certified). You will not be charged at the time of booking. Your card is held on file and only charged after your visit is completed, or in the event of a late cancellation or no-show as described below.
All services are charged at the rates posted on our website at the time of booking. Posted prices may change with notice. The price you see at the time of booking is the price that applies to your visit, except where insurance benefits change the patient responsibility amount.
If you are uninsured or choose not to use insurance, we will provide a Good Faith Estimate of expected charges before your visit, in accordance with the federal No Surprises Act (45 CFR 149.610). The estimate is delivered by email and saved to your patient record.
You can update your saved card, view your past charges, and download receipts at any time via the Stripe-hosted billing portal linked from your booking confirmation email. The portal is HIPAA-compliant and encrypted.
The short version:
✓ Cancel up to 1 hour before your appointment — no charge
✗ Cancel within 1 hour of your appointment, or don't show — $45 fee
★ Your first late cancellation or no-show is forgiven, no charge
↻ Subsequent $45 fees become $45 of store credit on your account, automatically applied to your next visit
You may cancel or reschedule your appointment at no charge any time up to one (1) hour before your scheduled visit. Cancellation links are included in every appointment confirmation and reminder email.
If you cancel within one (1) hour of your appointment time, or fail to show up for your scheduled visit, a $45 late cancellation / no-show fee will be charged to the card on file. This fee covers the lost provider time and the operational cost of the missed slot.
Every patient is granted one (1) lifetime waiver of the $45 fee. The first time you late-cancel or miss a visit, the fee is automatically forgiven and no charge will appear on your card. You will receive a friendly reminder of the policy after the waiver is used.
For all subsequent late cancellations and no-shows after your one-time waiver, the $45 fee charged to your card is immediately converted to $45 of store credit on your patient account. This credit is automatically applied as a discount on your next visit, so the fee is functionally non-punitive over time — it simply pre-pays a portion of your next visit. Credits do not expire.
The free cancellation window still applies to same-day bookings. If you book a visit less than 1 hour before its start time and then cancel, the late cancellation fee may apply unless your one-time waiver is still available.
Rescheduling more than 1 hour before your visit is treated as a free cancellation followed by a new booking — no fee applies. Use the reschedule link in your confirmation email or call us at (832) 205-8404.
Our telehealth services comply with Texas Occupations Code Chapter 111 and 22 Texas Administrative Code Chapter 174. By using our telehealth services, you acknowledge that:
Telehealth visits may be transcribed in real time by an AI-assisted clinical scribe to help with documentation. The provider reviews and edits all AI-generated notes before they are added to your medical record. You consent to this transcription as part of your Telehealth Informed Consent at intake.
If you provide insurance information at booking, we will verify your eligibility and benefits with your insurance company before your visit. Verification results, including any patient responsibility (copay, deductible, coinsurance), will be shared with you when available.
You are responsible for any amounts not covered by your insurance, including:
If your insurance denies a claim, we will notify you and bill you directly for the patient responsibility amount.
Our providers prescribe medications when clinically appropriate. Prescriptions are sent electronically to your pharmacy of choice via DxScript e-prescribing.
For controlled substances (DEA Schedule II–V):
We do not prescribe controlled substances on first visits to new patients without proper evaluation, regardless of your request.
Information provided on our website, in marketing materials, or in general communications is for informational purposes only and does not constitute medical advice. Medical advice can only be given by a licensed provider during a clinical visit, after evaluating your specific situation.
Individual results from any treatment vary. We make no guarantees about specific outcomes. Weight loss, hormone therapy, aesthetic, and other elective service results depend on factors including your physiology, lifestyle, and adherence to the treatment plan.
By booking elective services such as aesthetics, IV therapy, or weight management, you acknowledge that you have been informed of the risks, benefits, and reasonable alternatives, and you consent to the proposed treatment.
All content on our website — including text, graphics, logos, the Copergrine name and brand marks, photos, and software — is the property of Copergrine Health & Wellness or its licensors and is protected by United States copyright and trademark laws. You may not copy, reproduce, distribute, or create derivative works without our prior written permission, except for personal, non-commercial use.
To the maximum extent permitted by law, Copergrine Health & Wellness, its providers, employees, agents, and affiliates shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or related to your use of our services, including but not limited to lost profits, data loss, or service interruptions, even if advised of the possibility of such damages.
Nothing in these Terms limits liability for medical negligence, gross negligence, willful misconduct, or any liability that cannot be excluded under applicable law.
These Terms are governed by the laws of the State of Texas, without regard to its conflict-of-law provisions. Any disputes arising out of or relating to these Terms or your use of our services shall be resolved exclusively in the state or federal courts located in Harris County, Texas, and you consent to the jurisdiction of those courts.
We may update these Terms from time to time. The "Last Updated" date at the top of this page indicates when the most recent changes were made. Material changes will be communicated to active patients via email at least 14 days before they take effect. Continued use of our services after changes take effect constitutes acceptance of the updated Terms.
